Hospitalization

  • Patients who are scheduled for inpatient admission will be informed of their admission date by the relevant hospital department via telephone or postal mail.
  • Complete admission procedures at General Reception desk 5 “Admission” by the time instructed on the scheduled admission date.

All patients being admitted to the hospital must bring the following

  • Patient registration card (shinsatsu-ken; smartcard format)
  • Health insurance card (even if your health insurance card was checked during an outpatient examination the same month, it is required again for inpatient admission)
  • Patient guarantor form (nyuin hosho-sho; if you are unable to provide this at the time of admission, please submit it as soon as possible following admission)
  • Personal seal (personal-use mitome-in accepted)

Applicable patients must bring the following

  • Certificate of Maximum Patient Medical Payment Amount (Gendogaku Tekiyo Ninteisho; for patients under the age of 70)
    (Please complete relevant procedures prior to hospital admission. Holders of the Special Medical Expense Recipient Certificate [Tokubetsu Iryo Jukyusha-sho] are excluded.)
  • Certificate of Elderly Patient Expense Support (Korei Jukyusha-sho; for patients age 70–74 who hold this certificate in addition to their health insurance)
  • Public Medical Expense Support Recipient Certificate (Iryohi Kohi Futan Jukyusha-sho)
  • Standard Medical Fee Reduction Certificate (Hyojun Futangaku Gengaku Ninteisho; for persons of municipality-tax-excluded households and other eligible recipients)
  • Special Medical Expense Recipient Certificate (Tokubetsu Iryo Jukyusha-sho)
  • Hospital discharge certificate (tai-in shomeisho; if you were hospitalized at another institution within the last 3 months for the same injury or illness)
  • Please inform a member of the hospital staff if you are the recipient of any public medical cost support system, including iryo-fujo medical aid, support for treatment of children with disabilities (ikusei iryo), support for self-reliance (kosei iryo), support for treatment of specified chronic childhood disorders/diseases (shoni mansei tokutei shikkan), etc.
  • Please inform a member of the hospital staff if you are being admitted due to a traffic accident, workplace injury or similar (even if the relevant application process is not yet complete).
  • If you need to cancel your scheduled hospital admission for personal reasons, please contact the outpatient section of the department that scheduled your admission.
  • If you need to change your hospital admission date for personal reasons, please contact the relevant hospital facility (building) as far in advance as possible.

If you have recently made any change to, renewed, or are currently in the process of changing or renewing your health insurance (card), medical aid receipt certificate or other such documentation, contact General Reception desk 5 “Admission” as soon as possible.

Hospital Discharge Procedures

  • Document-related procedures for hospital discharge will be completed by your doctor(s) and nurse(s).
  • You will receive a bill for hospital fees (shinryo ryokin seikyusho) by your date of discharge informing you of hospitalization fees. Pay the indicated amount using an automated payment machine or at General Reception desk 4 “Payment”. Please note that additional fees may be charged at a later date for clerical operations and so forth following hospital discharge. Patients who plan to pay their hospital fees at a later date or are unable to pay on the day of discharge will be required to fill out and sign a confirmation of unpaid hospital bill and payment pledge document.
  • Make sure you take your patient registration card (shinsatsu-ken), follow-up examination reservation form, medications and so forth with you when you leave the hospital.
  • Also, make sure you have not forgotten anything in/on your assigned locker, refrigerator, bedside table, etc.

Hospital Fees

  • Hospital fees will be calculated at the end of each month and a bill sent around the 10th of the following month. Complete payment by the payment deadline indicated on the form.

  • You will receive a bill for hospital fees (shinryo ryokin seikyusho) by your date of discharge informing you of hospitalization fees. Pay the indicated amount upon discharge. Patients who plan to pay their hospital fees at a later date and patients who receive their bill at a later date will be asked to fill out additional paperwork. Please note that additional fees may be charged at a later date for clerical operations and so forth following hospital discharge.
  • Payment processing locations and hours are as specified below. Either cash or credit card may be used to make payments.
    Locations: At an automated payment machine or General Reception desk 4 “Payment”.
    Hours: Between 8:30 a.m. and 5:15 p.m. on weekdays (not available on weekends and national holidays)
  • Medical fee payment receipts are required for reimbursements on high-cost medical fees, income tax return medical cost exemptions (totaled for the January–December period), and other such procedures, so please keep careful track of them. Receipts cannot be reissued.

  • Patients who wish to pay medical fees via bank transfer can transfer their payment to one of the hospital’s bank accounts at The Iyo Bank, Ehime Bank, Ehime Shinkin Bank or Japan Post Bank. Patients are responsible for all bank transfer processing fees.
  • For questions regarding hospital fee payments, please inquire at General Reception desk 5 “Admission” before making your payment.
  • A detailed statement of hospital fees (iryohi meisaisho, one issued per hospitalization) will be provided along with the hospital fees bill.

Certificate Issuance

  • If you require the issuance of a certificate(s) by the hospital, please ask at General Reception desk 10 “Medical Certificate” on your day of discharge.
    (Patients discharged on a weekend or holiday are asked to request certificates on the weekday preceding discharge.)
  • For issuance of a certificate(s) after your date of discharge, please ask at General Reception desk 10 “Medical Certificate”.

Important Notes for Persons Visiting Patients

Visitation Hours

  • 2:00–7:00 p.m.
  • During visits, use the dedicated visitation area.
    Please avoid visiting outside of normal visitation hours, as this may interfere with patient care.
  • When visiting a patient, please go to the staff station first.
  • In order to prevent disease infection, avoid bringing small children during visitation.
  • Avoid visiting in large groups, as this may inconvenience or upset other patients.
  • Visitors may be refused visitation depending on the patient’s condition.
  • Eating and drinking during visits is strictly prohibited.
  • During visits, use the dedicated visitation area.
